Crisis Case Manager in Bracebridge

The role requires a Bachelor’s degree in a relevant field and one year of experience in crisis intervention, case management, or supportive counseling. You'll collaborate with a multidisciplinary team to develop opportunities for clients and build community support networks across the District of Muskoka. The ideal candidate will possess excellent communication skills, emotional intelligence, and a dedication to client success in their daily environments.
Key Responsibilities:
• Perform case management and psychosocial rehabilitation
• Assess, plan, and coordinate treatment for clients
• Engage with community landlords and service providers
• Support individuals in developing new skills
• Manage relationships with empathy and conflict resolution
Requirements:
• Bachelor's degree in a relevant discipline
• One year experience in mental health support
• Ability to work independently and in a team
• Familiarity with the Residential Tenancy Act
• Valid Ontario driver's license for client transport
