As a Clinical Case Manager with Kinark Child and Family Services, you will play a vital role in an interdisciplinary treatment team. You’ll facilitate client services through effective case management and collaboration with various stakeholders. This position requires your expertise in crisis intervention and mental health assessments to guide service delivery.
Key Responsibilities:
• Complete safety planning and risk assessments
• Support and assess active clients alongside therapists
• Coordinate and participate in case conferences
• Manage treatment team meetings and documentation
• Collaborate with community resources and stakeholders
Requirements:
• Bachelor’s Degree in Social Work or a related health discipline
• Must hold valid registration with relevant Ontario colleges
• At least 3 years of experience in mental health
• Strong skills in relationship building and crisis management
• Valid Ontario driver’s license and vehicle access
