At AMIkids Polk, we provide a safe and supportive home-like environment for up to 24 young men ages 10 to 16. Our focus is on fostering positive relationships, providing comprehensive case management, and offering a range of therapeutic services designed to equip these young men with the skills and tools they need to thrive.

Position Summary

The role of the Transition Coordinator is to oversee the transitional process of each youth entering and exiting the program/school. Manage the reentry of youth into society as a responsible, self-directed, individual actively engaged in the pursuit of vocational or educational goals.

  • Develop and maintain educational and vocational opportunities for youth,
  • Ensure the safety and supervision of all youth in accordance with all operational and contractual requirements,
  • Serve as advisor and positive role model for youth in the program,
  • Assign each new youth an advisor upon admission,
  • Develop, sign and maintain Performance Contracts for each youth, and revise Performance Contracts when necessary,
  • Conduct a comprehensive orientation program for all new youth,
  • Provide guidance to youth in the area of educational and vocational goals and objectives,
  • Ensure youth receives academic, vocational, and behavioral assessments within the required timeframe of enrollment and that this data is used in development of the individual service/care plan,
  • Schedule and conduct guidance sessions with youth, team members, parents and families ,
  • Ensure daily log documentation is completed,
  • Ensure proper youth medication administration and maintain required documentation,
  • Prepare or assist in the preparation of required youth reports,
  • Work closely with external partners,
  • Communicate directly with appropriate team members and supervisors concerning transitional information,
  • Assist in scheduling all required transitional meetings,
  • Ensure each youth has a post-needs assessment and has a completed transitional plan before release date,
  • Conduct home visits when necessary and based on youth needs,
  • Prevent and intervene in youth altercations and incidents, using the authorized state mandated training methods,
  • Administer first aid and CPR during emergencies in accordance with the national standards,
  • Drive program’s vehicles as required in accordance with AMIkids transportation guidelines,
  • Assist with special projects and other duties as assigned.
Minimum Education, Training and Experience
  • Bachelor’s degree in the field of counseling, social work, psychology, special education or in a related human services field,
  • Evidence of successfully securing community resources is preferred,
  • Minimum one (1) year of related experience working with youth is required, working with at‑risk kids a plus.
