Join Yes Care as a Utilization Manager Licensed Practical Nurse (LPN) and play a critical role in delivering compassionate and effective patient care within our dynamic healthcare program.
Key Responsibilities- Patient Assessment and Care Planning: Assist in assessing the health status of patients and contribute to the development and modification of individualized care plans.
- Direct Patient Care: Provide safe, effective nursing care for multiple patients within the facility’s healthcare program.
- Implement patient education initiatives: Based on established teaching plans.
- Nursing Scope of Practice: Perform delegated nursing tasks consistent with educational preparation and within the scope of practice. Use a problem‑solving approach to make informed clinical decisions.
- Care Coordination and Monitoring: Oversee onsite and offsite patient scheduling in collaboration with the care team. Ensure follow‑up care for patients, including telemedicine appointments and consult processes in accordance with policy and contract requirements.
- Quality Improvement and
Collaboration:
Participate in Quality Improvement initiatives. Coordinate with Regional Medical Director, site medical providers, Regional Nursing Director, and other site nursing leaders to ensure seamless patient care. - Documentation and Other Duties: Monitor CARES and ensure proper coordination as needed. Perform additional nursing duties as assigned to support patient outcomes and organizational goals.
- Educational Background: Graduate from an accredited Licensed Practical Nursing program.
- Licensure and Certification: Current licensure as a Practical Nurse (LPN) in the state of employment. Active CPR certification.
- Skills and Knowledge: Familiarity with state laws and regulations governing LPN practice. Successful completion of initial and annual clinical competencies to demonstrate required skills for the role.
