Das Claraspital in Basel sucht eine/n Sozialarbeiter/in, der/die eine individuelle Beratung und Information für Patienten und Angehörige gewährleistet. Zu den Aufgaben gehören die Austrittsplanung, die Klärung sozialrechtlicher Ansprüche sowie die Unterstützung bei der sozialen Reintegration im Rahmen einer interdisziplinären Zusammenarbeit.
