Role OverviewThis role involves supporting clients transitioning from hospital to their homes, providing flexible assistance for individuals with enduring mental health needs on a 7‑day shift rota. The focus is on guiding clients through their recovery journey over a 24‑month period, helping them set and achieve goals, become more self‑reliant, and reduce dependence on medical interventions and hospitalisations. The role also includes providing outcome‑focused support, addressing dual diagnosis issues, managing housing‑related aspects, and collaborating across various services and sites as needed.
Responsibilities- Support clients transitioning from hospital to home
- Provide flexible assistance for individuals with enduring mental health needs
- Guide clients through recovery over a 24‑month period, helping them set and achieve goals
- Work towards increased self‑reliance and reduced medical intervention and hospitalisation
- Provide outcome‑focused support and address dual diagnosis issues
- Manage housing‑related aspects and coordinate with other services and sites
The working pattern is a 2‑week rolling rota with five early shifts of 08:00-16:00 and five late shifts of 13:30-21:30, including one weekend shift every two weeks. Alternative shifts may be required to meet business needs.
