Join Our Team as an Associate Practitioner

An exciting opportunity has emerged within the Mid and North Hampshire Division to appoint a passionate and proactive Associate Practitioner in the North and West Hampshire Community Learning Disability Team.

This role is ideal for someone with experience supporting individuals with learning disabilities who is eager to make a meaningful impact. You will work directly with people with learning disabilities, their families, and carers – helping to shape and deliver compassionate, person‑centred care to improve their health outcomes.

What You Will Be Doing

Your day‑to‑day work will be varied and rewarding, including:

  • Conducting behavioural observations in home settings
  • Providing training and role modelling for staff
  • Advocating for reasonable adjustments to ensure equitable healthcare access
  • Creating personalised, accessible information and monitoring tools
  • Assisting with hospital admission planning and support to optimise care and ensure health investigations/interventions are successful

You will collaborate closely with our nursing team, contributing to assessments, care formulations, and interventions. You will also monitor individuals on our waiting list, ensuring timely updates on changes in their presentation or needs.

Main Duties of the Job

Where You Will Be Based

Our service operates from Winchester, Basingstoke, and Totton, with your primary base in Winchester. You will work alongside nurses within a multi‑disciplinary team delivering services throughout Mid and North Hampshire.

Support and Development

You will receive regular supervision and guidance from experienced nurses within our Learning Disability Service. We are committed to your growth, offering opportunities for continued professional development, reflective practice, and cross‑disciplinary collaboration.

What We Are Looking For

We are seeking someone who is:

  • Empathetic, organised, and self‑motivated
  • Skilled in communication, presentation, and interpersonal engagement
  • Insightful about the needs of people with learning disabilities
  • Competent in computer use, including Word, PowerPoint, Excel
  • A car driver, due to the geographical scope of the role
