Team Leader

We are recruiting a Band 7 Team Lead to lead a newly established Community Rehabilitation Team (CRT) within the Mid and North Hampshire Division, shaping a community-based rehabilitation service supporting adults with complex mental health needs to achieve recovery and independence.

Responsibilities
  • Provide leadership to a multidisciplinary team delivering rehabilitation interventions.
  • Ensure consistent delivery of care aligned to evidence-based practice and NICE guidance.
  • Oversee service performance, including waiting times, caseloads and outcomes.
  • Support smooth transitions between inpatient and community services.
  • Lead collaboration across system partners to improve pathways and integration.
  • Drive quality improvement, audit and service evaluation activities.
  • Maintain oversight of safeguarding and clinical risk within the service.
  • Provide clinical and operational leadership for the CRT across Mid & North localities.
  • Lead the delivery of a NICE-compliant rehabilitation service for individuals with complex psychosis.
  • Oversee team performance, ensuring high standards of quality, safety and access.
  • Maintain a reduced but complex caseload, supporting service users with significant rehabilitation needs.
  • Coordinate multidisciplinary working, ensuring effective care planning and risk management.
  • Support the development of integrated pathways with primary care, acute services, social care and VCSE partners.
  • Lead on demand, capacity and flow, ensuring effective use of workforce and resources.
  • Oversee referral management and decision-making processes.
  • Contribute to service development, transformation and improvement programmes.
  • Apply strong leadership and decision‑making skills.
  • Manage complex clinical and operational demands.
  • Communicate effectively and partner with teams across the system.
  • Leverage understanding of recovery‑focused rehabilitation and complex psychosis.
  • Analyse data and use performance information to drive improvement.
Qualifications
  • Professional qualification in Nursing, Occupational Therapy, Social Work or related discipline.
  • Current professional registration.
