Calling Band 6 Community Mental Health Practitioners!

Based at Parklands House, London Road, Basingstoke, the Older Persons CMHT offers assessment, diagnosis, treatment and support for people with a deterioration in cognition and/or dementia and over 65s who present with functional mental health illnesses. We work closely with carers, other services such as Adult Health and Social Care and the voluntary sector.

Overview

The CMHT is a lively, dynamic and supportive multidisciplinary team that provides care coordination and recovery‑focused care for individuals with complex and challenging mental health needs.

Responsibilities
  • Co‑ordinate the care of a complex caseload of service users while supervising a Band 5 Community Practitioner and a Band 3 Community Health Care Support Worker.
  • Formulate care and risk management plans in accordance with identified need, ensuring these are recovery‑focused and developed in collaboration with the service user and, where appropriate, their relative and carer.
  • Actively consider and participate in team development.
  • Carry a caseload and participate in the team's duty system.
  • Work alongside other agencies to promote holistic, recovery‑focused care; produce reports, supervise staff and run clinics as necessary.
Qualifications and Skills
  • Band 6 Community Mental Health Practitioner.
  • Excellent assessment and communication skills.
  • Sound clinical skills with the ability to coordinate complex care.
  • Good administration and IT skills.
  • Proficiency in report writing and organisational skills, with working knowledge of safeguarding.
  • Driving licence suitable for use in the United Kingdom and access to a vehicle for work purposes.
  • Experience working with older adults, dementia and functional mental health illnesses.
Working Hours

Monday to Friday, 9:00 to 17:00.
