Responsibilities Person-Centred Support- Actively involve younger individuals in planning and reviewing their support needs and goals, ensuring their support plan reflects their preferences.
- Empower each younger person to make choices and maintain as much independence as possible, supporting decision-making in all areas of life.
- Assist with personal care activities, such as bathing, dressing, toileting, and mealtime support, with respect for each person’s comfort and dignity.
- Support individuals with shopping for personal items, clothing, groceries, and assist with meal planning and preparation.
- Assist with medication management and administration when required, maintaining accurate records as needed.
- Encourage and support healthy eating, hydration, and meal preparation according to each individual’s dietary preferences.
- Facilitate involvement in social, recreational, educational, and employment opportunities based on the individual’s interests.
- Support with travel arrangements and accompany individuals on outings, holidays, and community activities as required.
