Community Care Live is recruiting for a qualified registered Social Worker to join the Basingstoke Hospital Social Work Team. You will work with health and social care colleagues to help adults leaving hospital achieve the best outcomes.
You’ll conduct comprehensive assessments, manage a complex caseload, supervise junior staff, triage safeguarding referrals and apply a strengths-based approach, with commitments to compassion and professional standards.
