. The Rehab Specialist will serve as a member of the Intensive Transition Team (ITT) for the State Hospital Stepdown Program, a new specialty transitional home designed to support persons dually diagnosed with behavioral health and intellectual developmental disabilities or autism. The program collaborates closely with Health and Human Services and all State Psychiatric Hospitals to build discharge plans for individuals who have been admitted to the state hospital for over one year.
The ITT identifies, assesses, facilitates step-down/transition, houses, and provides services to maintain community integration.
- Conduct a comprehensive pre-transition assessment for individuals referred to the program, considering intellectual and developmental, medical, substance use, and mental health conditions.
- Provide pre-transition services while the individual is in the state hospital for up to six months.
- Develop and facilitate implementation of the Individual Transition Plan (ITP) and Person Centered Recovery Plan (PCRP), addressing clinical and housing needs.
- Participate in the individual’s care throughout enrollment in the program, providing case management, skills training, group psychoeducation, and crisis intervention services.
- Manage crises and support cross-training for other programs, including the Outpatient Biopsychosocial Approach for IDD Persons (OBI) Program and Mobile Crisis Outreach Team.
Shift: 4 10s schedule. Options:
Sun-Wed 8a-6p or 10a-8p;
Wed-Sat 8a-6p or 10a-8p.

Salary: $24.23 – $36.48/hour
Monthly incentive: $200
Credentials, Qualifications, and PreferencesMinimum Qualifications: Bachelor’s degree in a social sciences/human services field, or a minimum of 24 college credits in a human services field with a completed Bachelor’s degree. Ability to be credentialed as a Qualified Mental Health Professional/Qualified Intellectual Disabilities Professional. Must have a valid Texas Driver’s License and maintain approved driver’s status during employment.
Preferred Qualifications: Two years of case management or crisis experience;
Master’s degree; pursuing clinical licensure in counseling, social work, or related field. Bilingual in ASL or Spanish preferred.
Salary Range: $50,398 – $75,878/year. Benefits package and leave accruals are described on the company website.
