** Job Description*
* The Habilitation Coordinator advocates for a vulnerable population of individuals in long-term care facilities. As part of the PASRR team, the Coordinator arranges services for adults with suspected mental illness or disabilities interested in community living options or requiring behavioral health services in nursing facilities.
Responsibilities include providing habilitation and IDD service coordination for Nursing Facility residents, assessing residents and new admissions to determine appropriate services or community placement, and collaborating with families and facility staff. The role requires assessments, person-centered recovery, and differential diagnosis skills. The caseload is approximately 25-30 individuals, with monthly face-to-face contacts documented in electronic health records.

** Salary Range:** $21.46 – $28.71/hour

* Minimum Qualifications:
*
* - At least one year experience with individuals with IDD
- Bachelor’s degree in a related field or 24 college credits in Human Services
- Valid driver’s license and good driving record

* - Experience with assessments and case management
- Master’s degree in social work, psychology, or counseling
- Bilingual in English/Spanish or ASL
- Knowledge of Medicaid Waivers
- Strong clinical judgment and computer skills
