Cheselden is an elderly care ward specializing in the rehab and care of patients with complex discharge issues.
As a Registered Nurse on the elderly care ward, you will play a vital role in delivering high-quality, compassionate, and person centered care to older adults, many of whom present with complex medical, cognitive, and social needs.
You will be responsible for assessing, planning, implementing, and evaluating individualised care plans, working as part of a multidisciplinary team to promote patient well-being, dignity and independence.
We re looking for an individual who has a caring, kind and professional approach, excellent clinical skills and the ability to respond to the changing needs of patients in a busy and sometimes challenging environment.

Main duties of the jobTo work according to the NMC Code of Professional Conduct and relevant professional guidelines as a named nurse or key worker (with facilitation) for a defined group of patients and take responsibility for:
- The assessment of care and health education needs
- The development, implementation and evaluation of programmes of care including discharge planning for each patient.
To gain experience and skills in:
- Clinical practice
- Facilitation and teaching
- Management (as appropriate)
To work in accordance with the RUH Nursing Strategy and contribute towards achieving its objectives and maintain effective communication and participate in clinical supervision as appropriate and to teach and act as a facilitator/mentor/preceptor/ role model to less experienced staff.

Person Specification Application form- Registered Nurses with NMC Registration
- Demonstrates interest in patient flow
- Show awareness of the best practice
- Demonstrate confidence in using electronic patient records
- Willingness to participate in service improvement
- Ability to mentor or support students nurses or junior staff.
- Experience in end-of-life care or advanced care planning
- Knowledge in age-related health conditions and their management, e.g. falls prevention; pressure ulcer prevention; polypharmacy)
- Evidence of ongoing professional development.
- Demonstrates compassionate; kindness and civility attitude
- Previous experience in elderly care
- Completion of relevant clinical skills training
