As a Care Coordinator you will be an integral part of our practice team, working closely with us within the (PCN) to support patient care. You will be practice based at Fairfield Park Health Centre and be integral to that team; but will work with other Care Coordinators within the PCN to develop best practice and shared learning.

Main duties of the job

This role requires you to deliver high quality levels of general administrative support to the practice, alongside specific duties to identify and manage a caseload of identified patients, ensuring that appropriate support is made available to them and their carers (if appropriate), and that their changing needs are addressed.

As a Care Co-ordinator, you will be an integral part of the Fairfield Park Health Centre team and will work closely with our GPs, Practice staff and other primary care colleagues in the practice and within the PCN to support patient care.

Bath Independents Primary Care Network (PCN) is a group of 3 practices which incorporate, Batheaston, Fairfield Park and Widcombe Surgeries. We deliver strong patient care to our PCN patient population by working collaboratively as a team.

This specific role would be based at Fairfield Park Health Centre who are a large forward‑thinking practice of 14,600 patients, based in a purpose‑built health centre on the north side of Bath.

Job responsibilities

Main Responsibilities

  • Undertakeadministrative work in line with PCN directed priorities
  • Proactively identify and maintain current cohorts of patients to support their personalised healthcare requirements
  • Ensurerregular and consistent communication with the clinical team regarding patientprogress and any complications or guidance. Including liaising with secondary care and other health providers
  • Raise awareness of health promotion, run campaigns, to include seasonal priorities.
  • Coordinateclinics, liaising with the reception lead and contacting the identifiedpatients with appointments
  • Managepatient initiated calls for help/signposting, ensuring patients are directed tothe appropriate service (eg GP, Nurse, First Contact Physiotherapist, Pharmacist, Social Prescriber)
  • Document and monitor aspects of patient coordination and service delivery, supporting data collection and audit using Systm One patient record system
  • Telephone contact to manage patients on the PCNs case load followingany unplanned hospital admission (where appropriate)
  • Participate in MDT meetings to discuss patients from the PCN case load
  • Raise awareness of health promotion and screening such as NHS Health Checks and LDHealth checks and coordinate invitations to ensure targets are achieved on apractice and PCN level
  • Assistand manage recall system for chronic diseases including acting as a point ofcontact for any queries regarding annual reviews
  • Actingas non clinical cancer Champion, coordinating referral practices, safetynetting and screening activity in conjunction with the practice clinical cancerlead
  • Ongoingmonitoring of PCN requirements including practice performance for IIF and QOF
  • Runaudits and searches where necessary to identify patients for review
  • Monitorreferrals to ensure tasks are completed and care delivered
  • Deliverreception services as needed by the Practice
  • Assistadministrative colleagues with various tasks as required (for example - patient registration/deduction)
  • Tocarry out any other reasonable duties as requested by a manager to ensure quality of service for both the PCN and Practice
Person Specification Qualifications
  • Experience of working within healthcare/voluntary/community sector.
  • Excellent interpersonal and negotiation skills.
  • Excellent written and verbal communication skills.
  • Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences.
  • Self‑motivating and able to work effectively as part of a team.
  • Experience of supporting people, families and carers in a related role (including unpaid work).
