The post holder will be a key member of the adult diabetes service, providing technical support and education to people with diabetes (PWD) using all forms of diabetes technology. The role is key to increasing digital access for all our patients and rolling out hybrid closed loop technology. This post holder will support the Diabetes Specialist Nursing (DSN) team by freeing up clinical time for diabetes management, reducing inequalities in healthcare and improving health outcomes.
Mainduties of the job
- To demonstrate and support the setup of glucose monitoring systems and associated readers, smartphone apps and online platforms as needed
- To support the DSNs with insulin pump training for PWD starting pumps, upgrades and initiating hybrid closed loop systems
- To facilitate or supervise the insertion of sensors/cannulas via sterile procedure
- To demonstrate how to download and synchronize devices for sharing clinical information
- Support diabetes clinics to ensure PWD attending have successfully shared clinical data prior to review
- To offer telephone, email, video call and face to face support and troubleshooting technology
- Providing assistance to housebound patient if indicated for technology initiation and intermittent downloads of glucose data
- To provide educational material and advice to PWD to ensure continuity of care
- To support PWD to access remote consultations where necessary
- To champion Personalised Care principles and practice in a patient centric way that supports physical and psychological wellbeing and facilitates patient self‑management.

Person Specification Qualifications and Training- Foundation degree in health and science care
- Nursing Associate Foundation Degree
- Experience of working with people with acute or chronic health needs
- Able to support, supervise and assess patient with Diabetes within clinical settings
- Ability to keep up to date with latest technology in Diabetes
- Experience of teaching
- IT literacy
