As an Advanced Clinical Practitioner

We are looking for an experienced and motivated Advanced Clinical Practitioner (ACP) to join the HCRG Care Group Inpatient Unit team. This is an exciting opportunity to work within a supportive multidisciplinary environment, delivering high-quality, patient-centred care across our community inpatient services.

Based at St Martin's Hospital, Bath
, you will provide advanced clinical assessment, diagnosis, treatment and management for patients requiring inpatient care. Working autonomously within your scope of practice, you will play a key role in supporting patient recovery and facilitating safe, effective care.

Hours: 37.5 hours per week

As an Advanced Clinical Practitioner , you will provide advanced clinical care to patients within our community inpatient rehabilitation services, primarily based at St Martins Hospital , with occasional cross-cover support at Paulton Memorial Hospital. You will assess newly admitted patients, formulate diagnoses and management plans, request and interpret investigations, and deliver high-quality, patient-centred care that supports rehabilitation and recovery.

Working autonomously within your scope of practice, you will manage complex clinical presentations and respond to acute health concerns that arise during a patient's inpatient stay. You will work closely with Consultant Geriatricians, specialist doctors, nursing teams, and therapists, contributing to ward rounds, multidisciplinary team meetings, and decision-making to ensure safe, effective, and coordinated care.
