Job IntroductionAs a Community Nurse Band 5, you will deliver high-quality, person centred nursing care to patients in their own homes, working in line with national and local policies, professional standards, and the NMC Code. You will work closely with patients, families, GPs, and multidisciplinary colleagues to assess needs, plan and deliver care, promote independence, and support positive health outcomes. You will contribute to service development, health promotion, continuous improvement initiatives, and the education of students and colleagues, ensuring the highest standards of care are maintained across the service.

Main Responsibilities- Assess, plan, deliver, and evaluate nursing care in partnership with patients, carers, and the multidisciplinary team.
- Monitor patient progress, recognising and escalating changes in condition appropriately.
- Prioritise and manage a caseload in line with patient need.
- Maintain accurate and timely patient records using electronic and paper-based systems.
- Deliver evidence-based care and contribute to quality improvement initiatives and clinical audits.
- Promote health, wellbeing, and independence for patients and carers.
- Participate in team meetings, handovers, clinical supervision, appraisals, and mandatory training.
- Support the learning and development of students and colleagues.
- Contribute to maintaining a safe environment through risk assessment, incident reporting, and adherence to infection prevention and control standards.
- Ensure the safe use, maintenance, and management of clinical equipment and stock.
You will be a NMC/1stlevel registered Nurse, you will have the ability to work both independently and also as part of a wider multidisciplinary team. It is expected that youshould have strong communication and organisation skills and a flexible approach which will ensureyou deliver positive experiences to patients at every stage of their healthcare journey.
You will have a full UK driving license with access to a car for work purposes with Business insurance for the vehicle used.
You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, Excel.
