Position: Early Help Family Practitioner

Help give children the best start in life. The first five years of a child's life are crucial in shaping their future health, development, and wellbeing. As an Early Help Family Practitioner, you will play a vital role in supporting children and families at the moments when early intervention can make the greatest difference. Working alongside Health Visitors, you will help deliver the Healthy Child Programme (0‑5), providing practical support, guidance, and early help to families across Bath.

Main Responsibilities
  • Carry out individual work with families as delegated by accountable Health Visitor and feedback outcomes.
  • Support the facilitation of local healthy family drop‑in hubs.
  • Plan and deliver universal 3‑4 month health promotion sessions, facilitating discussions with groups of parents/carers.
  • Undertake Universal mandated reviews, including 6‑8 week, 10 month and 24 month reviews as delegated by accountable Health Visitor, and feed back regarding any child or family requiring further assessment or referral to the named Health Visitor.
  • Carry out maternal mental health screening at 6‑week and 10‑month reviews using evidence‑based screening tools.
  • Participate in CO monitoring at key mandated contacts as per the CO monitoring pathway.
The Ideal Candidate
  • A National Nursing Education Board (NNEB) qualification or equivalent to BTEC (National Diploma in Nursery Nursing), or NVQ level 3 (Childcare and Education) or a Foundation Degree in Early Years/Education.
  • A full UK driving licence and access to a vehicle for work purposes.
  • Good communication and interpersonal skills, with the ability to work flexibly and adapt according to service needs.
  • A sound understanding of child development, parenting and the ability to apply behavioural programmes.
