Nurse Case Manager for Patient Services

Become a key player in patient support as a Nurse Case Manager. Assist patients and healthcare providers with therapy access and provide critical information on various health conditions in a meaningful role.

In your capacity as a Nurse Case Manager, you'll engage with patients and their families to provide assessments and necessary follow-ups. Your responsibilities include documentation, liaising between patients and healthcare providers, and participating in specialized training on therapies. You will work on ensuring patients receive timely support and resources throughout their health journey.

Key Responsibilities:

• Provide information regarding therapies and disease states

• Conduct assessments and support patient documentation

• Track and coordinate patient access to therapies

• Collaborate with healthcare professionals for logistics

• Monitor patient journeys through proactive follow-ups

Requirements:

• Diploma in healthcare or related field required

• Licensed Nurse (RN or RPN/LPN) with active registration

• 2-5 years of experience in healthcare settings

• Case management experience preferred

• Fluency in French and/or English may be required
