Nurse Practitioner at Thames Valley Health

Thames Valley Family Health Team seeks a Nurse Practitioner (MRP) in London, Ontario. Join a dedicated primary care team focused on patient-centered health services and care delivery.

In this role, the Nurse Practitioner will provide comprehensive care to a defined patient panel and work collaboratively within a multidisciplinary primary care setting. Preferred candidates should have two years of NP experience and a strong background in primary health care nursing, health promotion, and patient education. This full-time position requires working onsite at 1051 Wonderland Rd. South and includes possible after-hours clinic availability.

Key Responsibilities:

• Provide primary care services across the patient lifespan

• Educate patients on health promotion and disease prevention

• Manage critical lab results and patient follow-ups

• Collaborate with interprofessional healthcare teams

• Utilize EMR and maintain patient records efficiently

Requirements:

• Current Nurse Practitioner registration in Ontario

• Two years of NP experience preferred

• Baccalaureate of Science in Nursing required

• Membership with RNAO or NPAO

• Proven community or primary care experience
