MDS Coordinator; RN​/LPN

Job Responsibilities:

• Complete SNF MDS assessments and transmit accordingly.
• Oversee assessment processes, set assessment schedules, and ensure assessments are done accurately and timely.
• Participate in discharge planning.
• Observe and report responses to interventions.
• Communicate with patients, families, caregivers, and healthcare team to promote optimal care.
• Complete documentation supporting services provided, meeting organizational, clinical, and reimbursement requirements.
• Follow infection control guidelines.
• Contribute to quality improvement initiatives.

Qualifications:

• Current RN or LPN license in Louisiana.
• At least 2 years of MDS experience in long-term care.
• Current CPR certification.
