Enrollment Representative - FMOL PACE Baton Rouge

• Screen potential new participants for PACE Baton Rouge/Lafayette and initiate the intake process.
• Conduct intake questionnaires, explain the program, and complete Inquiry Screening Forms.
• Schedule and coordinate intake activities, including on-site and home visits as needed.
• Ensure residents have all necessary enrollment information and prepare them for the assessment process.
• Communicate with hospitals, nursing homes, physicians, and other community agencies to follow up on intake records.
• Serve as an integral part of the Interdisciplinary Team, providing information to staff for enrollment decisions.
• Complete enrollment activities and obtain necessary signatures, collaborating with Medicaid and required documentation.
• Maintain intake/enrollment documents and enter data into the electronic medical record.
• Comply with State Medicaid requirements for initial and annual applications within requested time frames.
• Initiate Medicaid applications, obtain verification, and correspond with OAAS/DHH to complete necessary documentation.
• Collaborate with Leadership and Marketing Specialist on marketing strategies and community outreach, including presentations and health fairs.
• Demonstrate advanced computer skills, correspondence skills, report writing, and telecommunications for board and leadership meetings.

• 2 years in healthcare, with at least 1 year working with frail/elderly population or a closely related care field.

• Associate degree (required).

• Multi-tasking, prioritizing tasks, creativity, computer skills, attention to detail.
