• Review, audit, and process OASIS documentation for accuracy, compliance, and completeness
• Ensure all OASIS data sets are completed and locked timely per CMS and agency standards
• Collaborate with clinical staff to clarify documentation discrepancies or missing data
• Verify appropriate ICD-10 coding and HHRG scoring in alignment with patient acuity and plan of care
• Utilize Homecare Homebase (HCHB) for workflow processing, QA, and documentation tracking
• Provide clinical and technical support to field clinicians for OASIS and HCHB-related questions
• Monitor and reconcile outstanding documentation to maintain timely billing cycles
• Support the case management team with coordination of care and interdisciplinary communication
• Maintain up-to-date knowledge of Medicare regulations, PDGM, and HHVBP requirements

• Minimum 3 years of experience in home health Quality Review
• Mandatory proficiency with HCHB (Homecare Homebase)
• Strong working knowledge of OASIS-E, PDGM guidelines, and CMS home health regulations
• OASIS CURRENT certification (COS-C or HCS-O) MANDATORY
  • Home Health Coding Certification HCS-D Preferred
• Detail-oriented, organized, and self-motivated
• Ability to work independently and meet deadlines in a fast-paced environment
