Patient Driver - PACE Baton Rouge Part Time
Our Lady of the Lake Health
The driver operates 14 passenger vans or smaller. The driver transports participants to and from their home to the PACE Center and to physician appointments. The driver prepares and provides appropriate documentation as required. The position relies on established guidelines to accomplish tasks and works under close supervision.
Responsibilities- Drive a specially equipped van to deliver physically limited patients/residents to varied destinations. Assure that wheelchair lifts and tie‑down equipment are properly operated and that patients/residents are delivered to appointments in a timely manner. Assure that all patients/residents are seated and have seatbelts properly fitted and secured.
- Follow established schedules, adjust schedules to meet unanticipated changes, determine the safest and most efficient pick‑up and delivery routes.
- Assist patients/residents on and off the van in a safe and courteous manner. Deal with medical equipment and other emergencies that may arise.
- Pick up secured medication from the pharmacy as scheduled and assist in delivery of sealed packaged medications to the nursing homes.
- Inspect van daily utilizing van checklist sheets. Report the need for maintenance.
- Maintain van in a clean, orderly and safe condition.
- Maintain accurate and timely maintenance and repair logs.
- Provide documentation of daily trip logs, mileage and other data required for DOTD monthly reports.
- Responsible for safe operation of the vehicle and keep the van with adequate fuel to avoid running out of gas while in transport with or without patients/residents.
- Attend staff meetings and take part in facility in‑services. May be required to perform Transportation Assessments.
- Communicate any change in patients/residents' condition to the nursing department.
- Complete documentation as required.
- Demonstrate willingness to assist staff with direct patient/resident care or activities as needed and as trained, when not driving the van.
- 5 Years of Good Driving Experience.
- High School Diploma or GED.
- Must be able to work effectively in a team environment and operate handicap accessible vehicles.
- LA CDL Driver's License (Class D or higher).
- BLS certification.
