The Program of All-Inclusive Care for the Elderly (PACE) is dedicated to enhancing the lives of older adults by providing comprehensive and coordinated care. Our services, many of which are offered onsite at PACE Centers, present an alternative to nursing home care. Through our interdisciplinary care team (IDT), which includes physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides, we deliver preventive, primary, acute, and long-term care, enabling older individuals to thrive in their communities.

As a Master Social Worker, you will play a critical role in coordinating the social service programs at Senior Care Partners P.A.C.E., in line with our regulations and established policies and procedures. Your work will directly contribute to the well‑being of our participants.
Key Responsibilities- Conduct in‑home comprehensive assessments and develop care plans.
- Manage community resources effectively.
- Collaborate with participants’ families and facilitate meetings.
- Act as an expert in Advance Care Planning.
- Provide education to alleviate caregiver burden.
- Education & Licensing: Must possess a Master’s Degree in Social Work (MSW) and be licensed in the State of Michigan.
- Experience: Minimum of one year of experience working with frail elderly or long‑term care populations.
- Skills:
- Detail‑oriented with strong problem‑solving abilities.
- Excellent written and verbal communication skills.
- Ability to maintain discretion and confidentiality.
- Strong organizational skills and proficiency in MS Office Suite (Word, Outlook, PowerPoint, Excel).
- Certifications: Current CPR certification by AHA, or willingness to obtain it through Senior Care Partners P.A.C.E.

- Customer Service: Provide exceptional support to all staff, external partners, and participants.

- Ability to cope with the mental and emotional stress of the position.
- Function independently and demonstrate flexibility and integrity.
- Capable of working effectively with participants, personnel, and support agencies.
- Must be able to push, pull, move, sit, or stand for long periods and lift up to 20 pounds as needed.
- Reliable transportation, current valid Michigan driver’s license, and car insurance.
- Must be 18 years of age or older.
- Must be fully vaccinated or willing to be fully vaccinated against Influenza, MMR, Varicella, Tdap/Td, Covid‑19, and Hepatitis
