The Program of All‑Inclusive Care for the Elderly (PACE) offers a variety of services, with many of them provided onsite at a PACE Center. PACE is an alternative to nursing home care and – through an interdisciplinary care team (IDT) of physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides – coordinates and provides preventive, primary, acute, and long‑term care services, so older individuals can continue living in the community.

Assures that participants will be assessed and triaged using the nursing process and approved guidelines to assist in obtaining the appropriate level of care and/or self‑care advice. Able to remain independent in the application of advanced nursing knowledge and skills with the ability to manage complex clinical situations and facilitate appropriate routing of concerns.
Duties and Responsibilities- Seek and obtain sufficient information necessary to effectively assess participants.
- Perform symptom management based on verbal or physical assessment. When appropriate, offer and complete a clinic or home visit.
- Assess and evaluate the participant's status by:
- Initiating orders for medications, durable medical equipment and diagnostics as prescribed by the Primary Care Physician or designee.
- Applying preventative and therapeutic nursing procedures and techniques as appropriate for the participant.
- Initiating or revising the plan of care.
- Conduct on‑call/emergent nursing assessments and scheduled visits when assigned and/or indicated to ensure the highest level of physical and emotional support for our participants.
- Additional nursing duties including but not limited to:
- Wound care, Foley care/insertion, IV antibiotics/fluids/TPN, phlebotomy, EKGs, foot/toenail care, medication administration, ear irrigation, and assisting with medical procedures, room participants and dressing/cleaning/transferring participants.
- Work in collaboration with participant/family/caregiver and the Interdisciplinary Team to provide continuity of care and meet the physical and emotional needs of the participant/family/caregiver.
- Must care, support, and collaborate with provider and members of the Interdisciplinary Team regarding ongoing patient needs and changes to the plan of care.
- Bachelor of Science degree in nursing preferred;
Associate’s degree required. - RN‑Nursing Diploma from accredited School of Nursing with licensure.
- Proficiency with MS Office Suite, Word, Outlook, PowerPoint, and Excel.
- Well‑developed ability to show discretion and maintain confidentiality.
- Effective communication skills, both written and verbal.
- Minimum of one year of experience working with the frail, elderly, or long‑term care population.
- Minimum of one year of home care experience preferred.
- Minimum of one year of wound care experience preferred.
- Must have reliable transportation, current valid Michigan driver’s license and car insurance.
- Must be 18 years of age or older.
- Must be fully vaccinated, or willing to be fully vaccinated against the following communicable diseases:
Influenza, MMR, Varicella, Tdap/Td, Covid‑19, and Hepatitis
