Home Health Occupational Therapist OT Part Time

Aveanna Healthcare is seeking a Part‑time Occupational Therapist (OT) to provide home‑health therapy services Monday‑Friday. The role involves assessment, treatment planning, and collaboration with interdisciplinary team members.

• Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current level of function.
• Administer, evaluate, and interpret tests to determine functional goals and outcomes.
• Provide written evaluations and plans of care defining OT needs, goals, progress, and expected outcomes.
• Deliver OT services per prescribed physician orders.
• Maintain plan of care via timely documentation; modify goals and treatment as needed.
• Plan for discharge, including educating patient and caregivers on home exercise plans.
• Coordinate services with health‑care team, physicians, parents/guardians, nurses, etc.
• Document according to policy with minimal supervision; avoid technical or clinical denials.
• Maintain effective communication among staff, healthcare team, and family; serve as resource consultant.
• Supervise other personnel (OTA, HHA) as applicable.
• Complete OASIS assessments per state regulation.

• Degree in Occupational Therapy from an accredited university/program.
• Current, unrestricted state license as an Occupational Therapist in the state of practice.
• Valid CPR and driver’s license.

• Experience in home‑health therapy and completion of OASIS documentation.

Minnesota, United States – Hourly rate: $50.00‑$60.00.
