Advisa Care is currently seeking dedicated and compassionate Home Health Registered Nurses to provide exceptional care to patients in their homes. Our mission is to enhance the quality of life for our patients while supporting them in their personal health journeys.

As a Home Health Registered Nurse with Advisa Care, you will be responsible for delivering skilled nursing care, implementing patient care plans, and collaborating with interdisciplinary teams to optimize patient outcomes.

• Assess patient health status and environment
• Perform complex nursing procedures and treatments
• Develop and implement individualized care plans
• Educate patients and families about health conditions and self‑care techniques
• Document patient care and progress accurately in accordance with policies
• Coordinate with physicians and other healthcare professionals to manage patient care needs

• Current and valid RN license in the state of practice
• Previous experience in home health or similar field preferred
• Strong assessment and critical thinking skills
  
  Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Valid driver’s license and reliable transportation
• Basic Life Support (BLS) certification
• Must be able to pass a drug screen and background check
