Family Peer Advocate – Youth ACT Team – NEW

Join to apply for the Family Peer Advocate – Youth ACT Team NEW role at Hope For Youth, Inc. This position supports children aged 10 to 21 who are in or returning from high‑intensity services, such as inpatient or residential care.

Organization Overview

For more than 50 years, Hope For Youth has provided services for children, youth, and families across Long Island, ranging from outpatient care to foster care and residential services. The program is licensed by the NY State Office of Mental Health and focuses on intensive, multidisciplinary interventions to facilitate successful community transitions.

Key Responsibilities
  • Follow program policies, employee manual, and state regulations.
  • Work within an interdisciplinary team to assist clients in achieving desired outcomes.
  • Provide rehabilitative support to parents or caregivers to benefit the child/youth.
  • Help families develop community connections and enhance quality of life in their own communities.
  • Teach parents strategies to support their child’s positive behavior and supply skill‑building training.
  • Attend trainings, professional development, daily team meetings, and family meetings.
  • Apply agency‑identified evidence‑based interventions best suited to meet the client’s needs.
  • Provide transportation for client to service locations as necessary.
  • Perform other tasks required to meet client, program, and agency needs.
Time Distribution
  • 60 Direct family therapeutic contact, including transportation
  • 20% Case documentation, reports, and case‑related meetings
  • 20% Staff/team meetings, supervision, and training
Qualifications
  • Family Peer Advocate Credential or willingness to obtain within a set timeframe.
  • Lived experience as a parent or caregiver to a child with a mental health diagnosis.
  • Computer proficiency, including Microsoft Office Suite, and experience working with EHRs.
  • High school diploma or GED.
  • Valid NYS driver’s license in good standing.
  • Physical ability to read/write reports, stand/walk for prolonged periods, bend, kneel, climb stairs, and drive own vehicle.
Employment Details

Full‑time, 35‑hour work week (flexible hours).

Location:

Central Islip, NY. Salary range: $20.00–$26.00 per hour.
