We’re looking for a Habilitative Coordinator/Case Manager to direct and coordinate the delivery of habilitative services – all while ensuring we provide a meaningful life in a caring community.

Responsibilities- Empowerment Enthusiast: You ll support and encourage individuals of all ages and abilities in making meaningful choices to determine their pathway in life.
- Compliance Coordinator: You ll coordinate the delivery of habilitative services, making sure we stay compliant with all regulatory requirements (you re our internal superhero!).
- Behavioral Trainer: Get ready to train our staff in behavior modification and/or intervention techniques.
- Program Pioneer: Manage the development of individual-centered programs and evaluate their effectiveness.
- Records Rockstar: Document individuals progress and maintain records like a pro.
- On-Call Lifeline: You ll be on-call to provide assistance and guidance to staff when those tricky problems pop up.
Schedule: generally days, salaried, on-call responsibilities to support the departments/agency which can include other hours (evenings, overnights, weekends) as needed.

Qualifications What You Need to Bring (Knowledge, Skills & Abilities):We re looking for someone who is patient, positive, and ready to adapt to changing demands!
- The Brainpower: Knowledge of habilitative services for individuals with developmental disabilities.
- The
Skills:
Excellent problem-solving, organizational, and planning skills. - The Training Touch: Skill in supervising and training staff in a positive manner.
- The Must-Haves: Ability to maintain confidentiality and accurate records.
- The Extra Edge: Skill in administering first aid and CPR.
? The Credentials (Education & Experience)
- Education: A Bachelor s Degree in Psychology, Special Education, or a related field.
- Experience: A minimum of three years of experience working with individuals with developmental disabilities is required.
- Certifications/Licenses: May require QIDP certification and experience in an ICF/, Med Aide
40, First Aid/CPR and Mandt. If you do not have these at hire, you must be willing to obtain them at the agency.
