Center Well Home Health, part of Humana, is seeking a Physical Therapist to deliver in-home therapy, helping patients regain strength, mobility and independence. You will work with a clinical team to plan and execute care in patients' homes.
Responsibilities include planning therapy, documenting progress, supervising PTAs and Home Health Aides, and coordinating with physicians. This is a full-time position with TB screening and driver-safety requirements, and a comprehensive benefits package.
