Best Hospice Care of Texas is looking for a compassionate and dependable Full-Time RN Case Manager to join our team!

As an RN Case Manager, you’ll manage a full patient caseload while working closely with patients, families, physicians, and our interdisciplinary team to provide compassionate, individualized hospice care.

• Manage and coordinate care for a full patient caseload
• Complete skilled nursing visits and ongoing patient assessments
• Develop and update individualized plans of care
• Provide education, guidance, and support to patients and families
• Collaborate with physicians and the interdisciplinary hospice team
• Maintain timely and accurate clinical documentation
• Participate in IDT meetings and on-call rotation as assigned
• Coordinate changes in patient condition and ensure continuity of care

• Current Texas RN license
• Compassionate, dependable, and organized
• Strong clinical assessment and communication skills
• Ability to independently manage and prioritize a full caseload
• Hospice or home health experience preferred
