We are seeking a Full‑Time Home Health LPN Visit Nurse to provide care in Fond du Lac and Dodge Counties

Asa Home Health LPN, you can expect:

- Provide quality care to home‑health patients, working closely with the RN case manager
- Provide wound care (we take pride in being a preferred provider for several local wound‑care clinics)
- Communicate effectively with clients, families, doctors, facility staff, and co‑workers
- Work independently with strong organization and time‑management skills
- Work with the geriatric population
- One year of clinical experience preferred
- Current driver’s license, valid vehicle insurance, and access to a dependable vehicle
Brown, Winnebago, and Calumet Counties
Schedule- Full‑time, 1st shift Monday‑Friday, minimal on‑call required
- $25‑$30 per hour based on experience
- Health, dental, vision, and life insurance
- Paid mileage for travel between patient visits
- Accrued paid time off (PTO) and sick time
