Eventus Whole Health delivers an integrated model of care to adults who reside in skilled nursing and assisted living facilities through a network of healthcare providers including Physicians, Nurse Practitioners, Physician Assistants, Medical Assistants and in‑house Support Staff.

Provide primary‑care services in accordance with Eventus Whole Health site‑specific protocols and consistent with the standard of care for the specialty of family medicine, geriatric medicine, and/or internal medicine as it pertains to long term care facility work.

• Review and sign MAR’s, orders specific for Primary Care, and manage all narcotic prescriptions per the Eventus Whole Health Narcotic Rx Policy. Review labs within 24 hours.
• Provide on‑call triage duties as per the on‑call triage protocol policy and adhere to the newest agreed triage schedule.
• Complete documentation and submit billing records according to published policies and provide the facility with a hard copy within 48 hours.
• Maintain open communication with facility staff and administration; provide verbal or written instruction and meet with Administrator or care coordinator prior to and after each visit.
• Adhere to Eventus Whole Health standards, policies, professional ethical standards, and all legal regulatory agencies' rules and guidelines.
• Maintain patient confidentiality, including proper use of cell phone, emails, text messaging, patient charts, and EMR; comply with HIPAA regulations.
• Consult with the clinical supervisor before refusing a resident, family, or facility request or recommendation regarding the care of a resident.

• Masters in Nursing
  :
  Accredited MSN and current valid state license issued by the Medical Board. All other necessary certifications must be current and valid.
• Physician Assistant
  :
  Graduate of an accredited PA program, passed national certification exam, licensed by state, and specialization in geriatric, family, or psychiatry desired.
