Primary Function

The Care Team Support Certified Nursing Assistant (CNA), based in the Primary Care department, will provide essential support to the clinic by handling patient phone calls, electronic in‑basket tasks, and other administrative responsibilities. This position will provide backup support for patient rooming when needed and work closely with other clinical team members to ensure timely patient care and seamless clinical workflows.

Duties & Responsibilities
  • Answer and manage patient phone calls appropriately and in a timely manner, following approved protocols.
  • Use the Trigger Situation Protocol to appropriately resolve and/or route calls, ensuring patient concerns are addressed in a timely manner.
  • Appropriately escalate calls to triage nurses according to the protocol, ensuring urgent matters receive prompt attention.
  • Provide in‑basket support for primary care. Handle a variety of in‑basket tasks efficiently and accurately, including reviewing and responding to MyChart messages, completion of provider assigned tasks, patient letters, expiring orders, etc. Prioritizing based on clinic need and urgency. Includes support for all primary care locations.
  • Collaborate with MAs and RNs in assigned pools to efficiently manage workload and ensure seamless patient care.
  • Patient rooming includes complete vital signs and standard rooming as described in the TPC rooming workflow. Medication review is excluded and will need to be performed by an LIP or Medical Assistant.
  • Schedule patient appointments.
  • Assist in the completion of referrals and prior authorizations in a timely and accurate manner, as needed.
  • Maintain proper and detailed documentation in the EHR, following clinic guidelines.
  • Work closely with clinic RNs, seeking their guidance on patient care situations and escalating issues appropriately.
  • Assist in quality improvement initiatives, identifying areas for system efficiency improvements and implementing solutions as directed by the supervisor.
  • Adhere to HIPAA regulations at all times, ensuring patient confidentiality and privacy.
  • Follow clinic safety standards, including proper use of safety equipment, knowledge of emergency procedures, and adherence to body mechanics and ergonomic practices.
  • Comply with all clinic policies regarding patient care, safety, and regulatory compliance.
  • Participate in clinic meetings, nursing department in‑services, and team training to enhance skills and contribute to team goals.
  • Provide support to team members, assisting with training and mentoring as needed.
  • Maintain clear communication with the supervisor, providing updates on workload, challenges, and patient care issues.
  • Maintain a high level of professionalism and confidentiality in all interactions with patients, providers, and colleagues.
  • Other duties as assigned.
  • Maintain regular, in‑person, work attendance and punctuality, as scheduled.
  • Requirements
    • Current CNA certification in the state of Oregon
    • High School diploma or equivalent
    • Current Basic Life Support (BLS) Certification
