Registered Nurse (RN) - $2500 Sign‑on Bonus!

- Provide care and services for a group of patients, visiting them in private homes, Skilled Nursing Facilities, and assisted living facilities.
- Act as a contributing member of the interdisciplinary team, offering expertise in psych‑social assessments, interventions, and community resource knowledge.
- Work in either a Case Management, Admissions, or Resource role.
- Current Registered Nurse (RN) licensure in good standing in the state(s) of practice.
- Works well within a team environment collaborating daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
- Works closely with patients on a 1:1 basis while supported by the patient care team.
- Can work independently and independently manage daily activities such as phone, e‑mail, written communication, meetings with various healthcare professionals, and a strong understanding of hospice rules and regulations.
- Time‑management skills to schedule visits, documentation, and meetings; prioritize and triage multiple concerns.
- Ability to learn an electronic medical records system.
- Willingness to drive with reliable transportation, a valid driver’s license, and auto insurance; able to travel across the service area.
