RN; Per Diem - PACE

This is an on‑call/per‑diem position working on weekdays, day shift and traveling to see patients.

The RN provides care management, care coordination, and skilled care for at‑risk frail elderly patients with complex medical and psychosocial issues.

Responsibilities include:

• Use population‑based disease management approaches and clinical protocols to support patient care.
• Work as a core member of the patient‑centered PACE interdisciplinary team (IDT).
• Assist providers in identifying patient needs and coordinating access to appropriate care and services to improve or maintain functional status and independence.
• Plan, oversee, and monitor all nursing and unlicensed caregiver services for each patient on the RN’s panel within the IDT context.
• Provide occasional local travel as required.

• Relevant coursework or training as specified by the Nurse Manager.
• Oregon Registered Nurse License upon hire.
• National Provider BLS certification (American Heart Association) within 30 days of hire.
• Relevant certifications applicable to the position, as specified by the Nurse Manager.

• Bachelor's Degree in Nursing.
• 1 year of home health RN experience.
