• The Advanced Practice Wound Provider reports to the Director of Clinical Practice and/or the Area Clinical Practice Manager.
• Collaborates on‑site with the Center Leadership Team for center‑specific goals and priorities.
• Works with the Medical Director and/or Attending Physician for patient‑specific medical care.
• Provides acute and chronic wound care services to nursing center residents in collaboration with other medical providers in the center.
• Supports the Center’s mission of improving lives through high‑quality health care and compassion.

• Graduate of an accredited School of Nursing program, licensed as a registered nurse.
• Certified Nurse Practitioner with a Master’s Degree in Nursing and concentration/specialty in Older Adult and/or Family Nurse Practitioner. Other concentrations considered based on experience and business need.
• Wound certification (CWS, WOCN, or equivalent) or actively pursuing such certification with APN wound experience.
• Minimum two years of wound care experience with adult or geriatric population preferred.
• Unrestricted APRN license and DEA registration for each state of practice; PA license and DEA registration similarly for physicians.

• Salary range: $123,000 – $127,700 per year.
• Sign‑on bonus: $5,000.
