This innovative facility integrates data and technology into patient assessment, care delivery, and communication, while providing specialized, person-centred support for individuals who no longer require hospital-level treatment but need additional care to safely return home or transition back into their community. Reporting to the Allied Health Manager, the Occupational Therapist plays an integral role in supporting patients’ goals of returning home or to their community. This role thrives on interprofessional collaboration, respectful communication, and evidence-based clinical expertise to develop care plans aligned with patient and family goals, using shared decision-making and mutual support.

About the Opportunity

As part of the allied health team, you will work in a highly collaborative and results-oriented environment alongside Physiotherapists, Recreation Therapists, Social Workers, Dietitians, Nurses, and Nurse Practitioners to support patients’ functional goals for discharge home.
