This innovative facility integrates data and technology into patient assessment, care delivery, and communication, while providing specialized, person-centred support for individuals who no longer require hospital-level treatment but need additional care to safely return home or transition back into their community. Reporting to the Manager of Transitional Services, the RN is integral in supporting the goal of returning to home/community by being engaged in interprofessional collaboration, offering evidence based clinical expertise to inform care plans that fit with patient/family goals, using effective communication strategies, shared decision-making, and mutual support.

Assesses, plans, implements, directs, and evaluates nursing care and functions, in accordance with the patient-directed approach to care through the established philosophy of service.Performs initial and regular health assessments to maintain patient quality of life.Assessing, planning, implementing and evaluating nursing care of all patients.Manages patient medication in accordance with legislative requirements.Follows the process in place for safekeeping of medicationsAdministers and records prescribed medications in accordance with facility policy.Ensures all completed care tasks are documented appropriately using identified technology and tools.Ensures Continuing Care Assistants/PSWs are completing care in a timely manner.Ensures Continuing Care Assistants/ document all completed care, incidents, and errors accurately.Applies dressings and treatments according to physician’s orders and facility policy.Development, implementation and evaluation of care plans.Observes, records and reports symptoms, reactions, treatments, and changes in the patient’s condition.Documents according to facility policy e.g Daily Patient Care Record, Medication Administration Record (MAR), patient progress notes, initiating medication incident reports, incident reports, and other facility specific documentation as required.Supports continuous quality improvement, risk management and quality assurance activities.Ensures organizational policies are adhered to by team.Ensures patient safety by ensuring team members follow and apply all standard operational procedures, safe work practices and training provided.Additional duties as requiredAbout You
