An NHS Trust in Bedfordshire is currently seeking an experienced Band 6 Occupational Therapist to support the Transfer of Care Team within the North Bedfordshire Therapy Service on a locum basis.
This role offers an excellent opportunity for an experienced Occupational Therapist to support safe and timely discharge from hospital, while also contributing to admission avoidance and improving patient flow across both acute and community services.
Role Overview
The successful candidate will work as part of the Transfer of Care Team, supporting patients across acute wards, community services, and rehabilitation pathways. The primary focus of the role is facilitating safe discharge through Pathway 1, ensuring patients are able to return home with appropriate therapy support and community services in place.
The post holder will also support urgent care response initiatives aimed at preventing unnecessary hospital admissions and contribute to the management of inpatient rehabilitation beds where required.
You will work closely with multidisciplinary teams including physiotherapists, nurses, discharge coordinators, social workers, and community services to ensure coordinated patient care and effective discharge planning.
Key Responsibilities
Provide specialist Occupational Therapy assessment and intervention across acute and community settings.
Support timely and safe hospital discharge, particularly through Pathway 1 discharge planning.
Work collaboratively with multidisciplinary teams to facilitate effective transfer of care.
Contribute to urgent community response services aimed at preventing hospital admissions.
Assess patients' functional abilities and recommend appropriate equipment, adaptations, and care support.
Support patient flow through proactive discharge planning and liaison with community services.
Deliver therapy interventions within inpatient rehabilitation beds where required.
Maintain accurate clinical documentation and adhere to NHS governance and professional standards.
Essential Requirements
HCPC Registered Occupational Therapist.
Experience working at Band 6 level or equivalent.
Experience with in acute hospital settings, discharge planning, or community rehabilitation services.
Knowledge of discharge pathways and patient flow processes.
Ability to work independently and effectively within a multidisciplinary team.
Strong communication and clinical reasoning skills.
Desirable Experience
Experience with in transfer of care or discharge coordination teams.
Knowledge of community rehabilitation pathways.
Experience supporting urgent care response services.
