The Memory Care Activity Aide is responsible for implementing individual and group activities based on residents’ needs and interests. The aide also provides patient/resident care as required under the direct supervision of the licensed nurse, and communicates with patients, residents, families and other health team members.
PRINCIPAL DUTIES AND RESPONSIBILITIES- Prepare and set up scheduled activities.
- Escort residents to scheduled day, evening, and weekend activities.
- Assist residents during activities.
- Transport and assist residents on ministry outings.
- Implement plan of care in relation to activities as appropriate to the level of living.
- Document and manage Activity Daily Sheets for activity participation.
- Participate in organizing and scheduling of community events.
- Implement plan of care developed by the Registered Nurse based on patient/resident physical, psychosocial, cultural, ethnic and religious/spiritual needs.
- Provide direct patient/resident care and report condition changes to RN or LPN.
- Assure resident safety by identifying safety hazards and emergency situations and initiating corrective action as appropriate.
- Document results of patient/resident care within established policies, procedures and scope of practice.
- Comply with all ministry policies and procedures.
- Perform other duties as assigned.

Typical physical demands include:
• Push and pull items such as resident/patient beds requiring up to 75 lbs.
• Carry or lift items up to 25 lbs. at waist height and occasionally above the head.
• Assist in maneuvering resident/patients weighing 100–400 lbs, and occasionally those over 400 lbs.
• Standing for extended periods.
