LPN 6P-6A – Catholic Care Center
Pay: $27.00–$32.00 per hour
$5,000 Sign‑On Bonus (limited time)
Shifts available: 6P‑6A. Pay today with onboard‑pay – early access to wages without interest or credit checks.
Principal Duties and Responsibilities- Perform patient/resident assessments and reassessments per policy.
- Develop, implement, and evaluate personalized plans of care based on assessment and diagnosis.
- Administer medications, demonstrating correct knowledge, dosage, and patient response evaluation.
- Educate patients/residents, families, and caregivers; document educational outcomes.
- Document clinical care according to current standards and policies.
- Plan discharge, anticipating long‑term and short‑term needs; collaborate with discharge planner.
- Delegate and collaborate with staff according to licensure and skill levels.
- Ensure resident safety by identifying hazards and initiating corrective action.
- Perform other assigned duties.
- Interpret policies for certified nursing assistants, medication aides, residents, and families.
- Supervise staff, evaluate direct care, and initiate corrective actions.
- Assign nursing duties with respect to staff qualifications and resident characteristics.
- Participate in employee evaluations, providing clinical performance information to the Director of Nursing or Deputy Director of Care Services.
- Monitor compliance with infection control, personal protection equipment, hazardous material handling, and fire safety.
- Ensure HIPAA compliance with resident and employee information.
- Completion of an accredited Registered Nurse program.
- Minimum one year RN experience preferred (not required).
- Supervisory experience preferred (not required).
- Knowledge of state and federal regulations.
- Current state RN license in good standing.
- CPR certification preferred for Assisted Living; required for LTC within 30 days of employment.

Physical Demands- Access all areas.
- Lift up to 50 lbs occasionally, 25 lbs frequently, and 7 lbs constantly.
- Assist with lifts >25 lbs.
- Move up to 25 lbs above head occasionally.
- Push/pull beds requiring up to 75 lbs.
- Assist patients up to 400 lbs.
- Constant hand grasp, fine dexterity; frequent standing, walking, pushing, bending, reaching, crouching, kneeling.
- Occasional stationary sitting.
- Stand/walk up to 8 hours on hard/soft surfaces.
- Verbal and written communication required.
- Auditory discrimination of voices and phone calls.
- Use of visual skills (close, distance, color, peripheral, depth).
- Detect unusual odors.
- Demonstrate honesty, integrity, caring, and motivational presence.
- Learn and apply Catholic Healthcare principles in decision making.
