Registered Nurse (RN) - Senior Behavioral Hospital (SBH)

Provides professional nursing care under the direction and supervision of a physician in a manner consistent with the philosophy and objectives of Catholic Care Center – Senior Behavioral Health Program. Coordinates and delegates nursing care using the nursing process integrated into the multidisciplinary treatment team plan of care, and is accountable for assigned nursing care activities on a shift basis, promoting and enhancing professional nursing practice on the unit.
RN staff may assume charge nurse responsibilities as required and works collaboratively with the health care team to maintain a safe therapeutic environment and provide compassionate person‑centered care.
- Provide direct patient care, assist with daily living activities, and support patients with ADLs.
- Lead and participate in program‑related activities and groups that reinforce daily goals, learned skills, and cognitive stimulation.
- Utilize trained de‑escalation techniques and adopt a trauma‑informed, person‑centered approach toward patients.
- Perform patient rounds and safety checks; promptly remove and report any safety issues.
- Perform and document vital signs and utilize appropriate patient care equipment.
- Follow unit safety practices and procedures, including hand hygiene, PPE, prevent falls, and contraband prevention.
- Act as charge nurse as required.
- Collaborate with the multidisciplinary team—medical, therapy, and support staff—to maintain therapeutic milieu and ensure safe, compassionate care.
- Provide patient and family education as part of program services.
- Receive, review, and complete shift hand‑off reports.
- Lead plan of care and carry out treatment plan objectives and interventions.
- Oversee and direct nursing care as needed.
- Administer medications accurately and per physician orders.
- Notify physician promptly of condition changes or clinical concerns.
- Complete shift assessments, document accurately.
- Engage with visitors and families to ensure unit safety.
- Monitor patient condition and respond per best practice standards.
- Supervise and assist LPNs and non‑licensed support staff to meet care standards.
- Obtain, transcribe, and execute physician orders.
- Perform medication administration, near‑patient testing, and review lab results.
- Report abnormal or critical lab values to physician.
- Ensure timely delivery of therapeutic programs.
- Adjust assignments and activities in response to changes in patient milieu.
- Complete precise documentation in the medical record.
- Complete admission and discharge processes.
- Provide patient education on medications and care, documented in record.
- Participate in treatment team meetings and quality improvement programs.
- Ensure ordered consults are completed within specified time frames.
- Assess and implement necessary interventions for fall prevention, suicide risk, and elopement prevention.
- RN license in the state of employment.
- Experience in behavioral health or psychiatric nursing preferred.
- Strong knowledge of evidence‑based practice and state nursing board regulations.
- Demonstrated ability to maintain a safe, therapeutic milieu and provide age‑appropriate care to older adults (55+).
- Excellent clinical judgment, patient‑centered communication, and teamwork skills.
- Ready to work collaboratively with multidisciplinary teams and adapt to a dynamic environment.
