- Rehab Vanrath are delighted to be partnering with HSCNI to recruit on their behalf an Occupational Therapist to join their Rehab Team (working with ages 65+ years) Main responsibilities will include:
To manage a caseload of patients with complex and multiple diagnosis and be responsible for organising own workload in line with agreed departmental priorities, policies and procedures. To be responsible for the assessment and treatment of patients referred; this will involve:
Comprehensive assessment of patients in all areas of occupational performance, demonstrating advanced clinical reasoning and analytical skill The establishment, implementation and modification of person-centred treatment programmes to address patients' complex and changing need
- To work with patients and carers, providing support and advice relating to practical management of the patient and providing up to date information to assist them in decision making To prescribe equipment or minor adaptations deemed necessary to facilitate discharge. To discuss assessment and treatment goals with patients and their carers communicating sensitive information in a clear, concise and empathetic manner.
To undertake specialist seating assessments including wheelchairs and pressure care assessments for patients as required. To carry out manual handling and risk assessments and in liaison with Team Leader/Advanced Practitioner Occupational Therapist implement subsequent actions. To utilise appropriate skill mix in delivering Occupational therapy interventions. Be responsible for maintaining accurate, contemporaneous clinical records in line with the Royal College of Occupational Therapists, Health and Care Professions Council and local standards of practice To communicate and work jointly with other members of the multidisciplinary team to ensure a co-ordinated approach to patient care.
To liaise and exchange information both informally and formally at huddles, white board meetings, best interests' meetings and MDT meetings, to ensure effective person-centred care planning and communication. To liaise with colleagues to ensure a seamless transfer from inpatient care to community-based services and, if required, make onward referrals or/and recommendations. To liaise with other professionals, private, voluntary & statutory agencies as appropriate.
Essential Criteria :
Occupational Therapy Degree (UK recognised) HCPC Registered Member of RCOT or Unison Mnimum previous 2 years OT experience Why choose Vanrath:
