• Lead and contribute to management of long‑term conditions:
• Asthma / COPD
• Diabetes
• Coronary Heart Disease
• Monitor chronic disease in line with protocols, including prescribing adjustments
• Arrange and interpret blood tests
• Deliver cervical screening
• Provide contraceptive services and HRT
• Deliver lifestyle advice and health promotion

• Provide day‑to‑day leadership and support to the nursing team
• Act as a role model for high clinical and professional standards
• Support staff development, supervision and mentorship
• Contribute to service planning and quality improvement initiatives
• Promote a positive, inclusive and supportive team culture

• Maintain NMC registration and adhere to the Code of Professional Conduct
• Maintain an up‑to‑date Personal Development Plan (PDP)
• Participate in clinical meetings and practice development
• Undertake any other reasonable duties as required
• Registered Nurse (NMC)
• Experience in primary care / chronic disease management
• Evidence of leadership or supervisory experience
• Cervical screening competence
• Strong communication and team‑working skills
