We’re looking for a licensed vocational nurse to support our IOP program, providing patient care under the supervision of a registered nurse. The role involves taking vital signs, administering medications, preparing patients for exams, collecting specimens, assisting with procedures, educating patients on self‑care, and documenting patient information.

• Participates in planning, coordinating, and documenting total patient care in conjunction with the multidisciplinary treatment team.
• Accurately reports and records the client’s condition and care, including signs and symptoms which may be indicative of change.
• Accurately monitors skin assessments with proper documentation and reporting of changes.
• Completes, maintains, and documents accurate and relevant clinical notes.
• Ensures timely and accurate clinical documentation for all nursing clinical services provided.
• Participates in education, in‑services, and other training and meetings as needed.
• Facilitates appropriate nursing action(s) in response to administrative directives and a changing healthcare environment.
• Ensures all MD orders are followed and documented accordingly during the assigned shift.
• Manages medication administration, monitoring, and proper storage of medication and complies with all medication management policies and procedures.

• Graduate of an accredited school of nursing.
• Must have an active Licensed Vocational Nurse license in the state of Texas.
• Minimum of 1 year experience as an LVN; experience in behavioral health care preferred.
• Certification in Basic Life Support.
• Certification in non‑violent crisis intervention.
