PT Registered Nurse

Responsibilities

• Using appropriate assessment tools/techniques to assess patients’ individualized needs
• Executing and continuously monitoring care plans
• Engaging patients using a collaborative approach by building rapport and trust
• Maintaining and updating professional patient records related to patient encounters and plans

• Registration with the CNO as a Registered Nurse in good standing
• Valid driver’s license and access to a vehicle is required
• Experience in delivering community care is an asset
• Reliability and flexibility
• Willingness to travel to and from patient residences within a designated area
• Passion for making a difference in your community
• Required on-call coverage, including after hours to ensure patient safety and to meet patient care needs
• Bilingualism (English/French) is preferred to support client care

Compensation will be discussed during the recruiting process. Base salary is one component of a broader total rewards package. Final salary offers are based on a variety of factors such as skills, experience, education, and alignment with responsibilities.
