Position: CDPM Nurse at BQWCHC - Full Time

Join the Belleville and Quinte West Community Health Centre as a Chronic Disease Prevention and Management Nurse. This full-time role focuses on educating and supporting individuals with chronic conditions.
As a CDPM Nurse, you will play a crucial role in providing individualized care for clients with diabetes, hypertension, and other chronic diseases. This permanent position allows you to deliver care through assessments, health education, and collaboration with an interdisciplinary team. The role aims to promote healthy living and address social determinants of health.

Key Responsibilities:

• Assess and triage clients based on nursing standards

• Offer counseling and education for chronic disease management

• Work with Nurse Practitioners on medical directives

• Provide backup for general clinic nursing functions

• Develop and evaluate health promotion programs
Requirements:

• Active registration as an RN or RPN with the CNO

• Three years of chronic disease management experience (RN)

• Background in community or primary care nursing

• Certification as a Certified Diabetes Educator preferred

• Experience in program delivery and evaluation
